Dear Sir/Madam,
If you are a holder of a savings and settlement account, savings account, or term deposit account, and it is not a joint account, we would like to inform you about the possibility of appointing a distribution of the funds accumulated in the above-mentioned accounts in the event of death by submitting a death disposition.
In the event of the account holder's death, the funds in the above-mentioned accounts will be paid out to the persons you specify. The death disposition can be submitted at any branch of our Bank. The rules for submitting a death disposition for the above-mentioned bank accounts are regulated by Article 56 of the Banking Law Act (Journal of Laws of 2015, item 128, as amended), with the following wording:
1. The holder of a savings account, a savings and settlement account, or a term deposit account may instruct the bank in writing to pay out from the account, after his/her death, specified amounts of money to persons indicated by him/her: spouse (ancestors – parents, grandparents, great-grandparents, etc.), siblings, (descendants – children, grandchildren, great-grandchildren, etc.) a specified amount of money (death disposition).
2. The amount of the payout referred to in paragraph 1, irrespective of the number of dispositions issued, may not exceed twenty times the average monthly salary in the enterprise sector, excluding profit-sharing bonuses, announced by the President of the Central Statistical Office for the last month before the death of the account holder.
3. The disposition for death may be changed or revoked in writing at any time by the account holder.
4. If the account holder has issued more than one death disposition, and the total sum of the dispositions exceeds the limit referred to in paragraph 2, the later disposition takes precedence over the earlier one.
5. The amount paid out in accordance with paragraph 1 is not included in the estate of the account holder.
6. Persons who have received amounts based on a death disposition in violation of paragraph 4 are required to return them to the heirs of the account holder.
Since July 1, 2016, in the event of the death of a bank account holder who has submitted a death disposition, the Bank is obliged to inform the persons indicated in the disposition about their right to the funds resulting from this.
Therefore, if you have submitted such a disposition at the Bank, we would like to remind you of the need to update the address data of the persons indicated in the disposition to ensure its smooth implementation.

The Cooperative Bank in Szczytno informs that it is refunding costs for consumer loans for individual clients under the principles defined by the ruling of the Court of Justice of the European Union in case C-383/18 dated September 11, 2019.
The refund procedure applies to loan agreements subject to the Consumer Credit Act, concluded since December 18, 2011 (i.e., after the Act came into force), not secured by a mortgage, in cases where the loan was fully repaid before the deadline specified in the loan agreement. Clients have the opportunity to receive from the Bank a proportional refund of the costs that constitute the “total cost of the loan” (within the meaning of Article 5 point 6 of the Act of May 12, 2011, on consumer credit), which the borrower was obliged to incur in connection with the concluded consumer credit agreement. The amount of the refund is proportional to the period by which the duration of the relevant loan agreement was shortened. The calculation of the aforementioned refund amount is carried out using the so-called “linear method”.
To obtain the aforementioned refund, consumers who have not yet received a Refund of Costs should submit an application to the Bank in one of the following ways:
